Transform NDIS Supports was not created from a business plan. It came from a mother who refused to accept that her son's life couldn't be better.

JACQUELINE'S STORY

How this business started.

When Jacqueline Machaka’s son was diagnosed on the autism spectrum at Level 3, he was two years old. He was non-verbal. He struggled to connect. He couldn’t function independently.

I did not hear the Psychologist words when she recommended the NDIS supports, my brain was racing on to what I could do to ensure he is functional and independent in this world. I just asked who and what will support this journey, she listed the therapies and by the next day I had an interim team, So Jacqueline built what he needed herself. A month later I went back and inquired what NDIS was, and was supported to complete the initial access paper work for NDIS. She researched therapy models, healthcare approaches, and support structures. She tested what worked and removed what didn’t. She built a network around her son from the ground up.

Over years, something shifted. He began to improve. The boy who hadn’t spoken began to communicate. The child who couldn’t function independently began to manage. By the time he was a teenager, he was functional, safe, and living his own life. The boy who spoke at 9 yrs old can read and write, loves coding and engineering aspects of life. Still building on his social skills and actually has friends who adore him.

She registered Transform NDIS Supports and held the audit during the Covid period started with support coordination. She was good at it. But one moment changed everything.

The moment that defined the business.

A hospital called. A participant was refusing to move to a facility. She was stuck in a bed, blocking a ward, and nobody was listening to what she actually wanted.

Jacqueline went. She sat down. She asked questions. She listened.

The participant didn’t want a facility. She wanted to go back home.

Jacqueline inspected the house, arranged a deep clean, secured sign-off from guardians, and built the support structure around what the participant needed to be safe there. She went home. She stayed home. She was fine.

Our team is made up of family members of NDIS participants, people with lived experience as participants, accredited and screened support workers, a dedicated care management team, and a leadership team delivering strategic, compliant and sustainable business practices focused on long-term participant care.

All our staff and contractors are trained in the NDIS Code of Conduct and are required to treat participants with dignity and respect, protect participant privacy and safety, act with honesty and integrity, and report concerns or incidents without delay.
